Empagliflozin, Metformin tablets 1000 mg, 12.5 mg What is empagliflozin, metformin and what are its indications: This medicine contains two active substances, empagliflozin and metformin. Each belongs to a group of medicines called "oral antidiabetics." These are medicines taken orally to treat type 2 diabetes mellitus. What is type 2 diabetes mellitus? Type 2 diabetes mellitus is a disease caused by your genes and your lifestyle. If you have type 2 diabetes, the pancreas does not produce enough insulin to control blood glucose levels, and your body cannot use its insulin effectively. This leads to high blood glucose levels which may cause medical problems such as heart disease, kidney disease, blindness, and poor circulation in your extremities. How empagliflozin, metformin works: • Empagliflozin belongs to a group of medicines called sodium-glucose cotransporter-2 (SGLT2) inhibitors. It works by blocking the SGLT2 protein in your kidneys. This results in removing sugar from the blood in the urine. • Metformin works differently to lower blood sugar levels, mainly by preventing glucose production in the liver. • This medicine lowers the amount of sugar in the blood. It can also help prevent heart disease. Why empagliflozin, metformin is used: • Empagliflozin, metformin is added to diet and exercise to treat type 2 diabetes in adults (18 years and older) who cannot control their diabetes by adding metformin alone or metformin with other diabetes medicines. • Empagliflozin, metformin can also be combined with other medicines for diabetes treatment. These medicines may be oral or injectable like insulin. • Additionally, this medicine can be used as a substitute for taking both empagliflozin and metformin as single tablets. To avoid overdose, do not continue taking empagliflozin and metformin tablets separately if you are taking this medicine. • It is important to continue your diet and exercise plan as instructed by your doctor, pharmacist, or nurse. What you need to know before using Trumpopeg Olamine Do not use Trumpopeg Olamine: • If you are allergic to empagliflozin or metformin or any other ingredients of this medicine. • If you have uncontrolled diabetes, such as severe hyperglycemia (very high blood glucose), nausea, vomiting, diarrhea, rapid weight loss, lactic acidosis, or ketoacidosis. Ketoacidosis is a condition where substances called "ketone bodies" accumulate in the blood and can lead to a pre-coma stage of diabetes. Symptoms include stomach pain, rapid and deep breathing, drowsiness, or unusual fruity odor in breath; • If you have pre-coma diabetes. • If you have severe kidney failure. • If you have severe infections such as lung, bronchial, or kidney infections. Severe infections can cause kidney problems, increasing the risk of lactic acidosis. • If you have lost a lot of water from your body (dehydration), for example due to prolonged or severe diarrhea, or if you have vomited multiple times in a row. Dehydration can cause kidney problems, increasing the risk of lactic acidosis. • If you are being treated for acute heart failure, recently had a heart attack, or have serious circulatory problems (such as shock), or breathing difficulties. This may lead to insufficient oxygen supply to tissues, increasing the risk of lactic acidosis. • If you have liver problems. • If you drink large amounts of alcohol, either daily or occasionally. Precautions and warnings: Risk of lactic acidosis Empagliflozin, metformin may cause very rare but very serious side effects called lactic acidosis, especially if the kidneys are not functioning properly. The risk increases with uncontrolled diabetes, severe infections, fasting for long periods or alcohol consumption, dehydration, liver problems, or any medical condition where part of the body suffers from oxygen deficiency (such as severe acute heart disease). • If any of the above applies to you, consult your doctor for further instructions. • Stop taking the medicine briefly if you have a medical condition that may be associated with dehydration (significant fluid loss) such as severe vomiting, diarrhea, fever, heat exposure, or if you are drinking less fluids than usual. Talk to your doctor for more instructions. • Stop taking the medicine and contact your doctor or nearest hospital immediately if you experience symptoms of lactic acidosis, as this condition can lead to coma. Symptoms of lactic acidosis include: • Vomiting • Stomach pain • Muscle cramps • General discomfort with severe fatigue • Difficulty breathing • Low body temperature and heartbeat Lactic acidosis is a medical emergency and requires hospital treatment. Children and adolescents: This medicine is not recommended for children and adolescents under 18 years old because it has not been studied in these patients. Other medicines and empagliflozin, metformin: • If you need to receive an iodine-based contrast agent intravenously, for example during X-ray or imaging tests, you should stop this medicine before or at the time of injection. Your doctor will decide when to stop and when to resume treatment with this medicine. • Inform your doctor if you are taking, have recently taken, or might take any other medicines. You may need additional blood glucose tests and kidney function tests, or your doctor may need to adjust your dose. It is especially important to mention: • Medicines that increase urine production (diuretics), as this medicine may increase the risk of significant fluid loss. Your doctor may ask you to stop taking this medicine. • Other medicines that lower blood sugar such as insulin or "sulfonylureas." Your doctor may want to reduce the dose of these medicines to prevent severe hypoglycemia. • Medicines that may alter the amount of metformin in the blood, especially if you have impaired kidney function (such as verapamil, rifampicin, cimetidine, dolutegravir, ranolazine, trimethoprim, vandetanib, itraconazole, crizotinib, olaparib). • Bronchodilators (beta-2 agonists) used for asthma. • Corticosteroids (taken orally, by injection, or inhaled), used for infections like asthma and arthritis. • Medicines used for pain and inflammation (NSAIDs and COX-2 inhibitors, such as ibuprofen and celecoxib). • Some medicines for high blood pressure (ACE inhibitors and angiotensin II receptor blockers). • Medicines containing alcohol (see "This medicine with alcohol"). • Iodine-based contrast agents (used during X-ray imaging). This medicine with alcohol: Avoid excessive alcohol consumption while taking this medicine, as it may increase the risk of lactic acidosis. How to use empagliflozin, metformin: Always take this medicine exactly as your doctor has told you. Consult your doctor or pharmacist if you are unsure. How much to take: • The dose varies depending on your condition and the current doses of your diabetes medicines. Your doctor will adjust your dose as needed and tell you exactly the strength of the medicine you should take. • The recommended dose is one tablet twice daily. Your doctor will usually start this medication by prescribing a tablet strength that provides the same metformin dose you are already taking (850 mg or 1000 mg twice daily), and the lowest empagliflozin dose (5 mg twice daily). If you are taking both medicines separately, your doctor will start treatment with tablets that provide the same total amount of both. If you have kidney impairment, your doctor may prescribe a lower dose. How to take this medicine: • Swallow the tablet whole with water. • Take the tablets with meals to reduce the likelihood of stomach upset. • Take the tablet twice daily by mouth. Your doctor may prescribe this medicine with another diabetes medicine. Remember to take all medicines as directed by your doctor for the best health outcomes.
