Tofflemire

Tofflemire هو نظام مصفوفة معدنية يُستخدم غالبًا في ترميمات الأسنان الخلفية، خصوصًا تحضيرات Class II. يتكوّن من حامل Matrix Retainer وشريط Matrix Band، ويعمل على تكوين جدار مؤقت حول السن المحضّر لحصر مادة الترميم، والمساعدة على إعادة بناء الجدار الجانبي للسن وتكوين Contact Point مناسب مع السن المجاور.

Tofflemire is a metal matrix system commonly used for posterior restorations, especially Class II preparations. It consists of a Matrix Retainer and Matrix Band. It creates a temporary wall around the prepared tooth to contain the restorative material and helps rebuild the proximal wall and establish an appropriate contact with the adjacent tooth.

طريقة الاستخدام بالتفصيل:

اختيار الـ Matrix Band: يختار الدكتور الـ Matrix Band المناسب لحجم وشكل السن، ثم يحدد اتجاه وضعه بحيث يكون الجزء الأوسع من الـ band باتجاه اللثة والجزء الأضيق باتجاه الإطباق.

تركيب الـ Matrix Band: يقوم الدكتور بإدخال الـ Matrix Band داخل الـ Tofflemire Retainer، ثم يضبطه بحيث يكون ثابتًا ويمكن التحكم في محيطه.

تجربة المصفوفة: يضع الدكتور الـ Tofflemire حول السن للتأكد من أن الـ Matrix Band يغطي المنطقة المفقودة بالكامل وأنه يصل إلى المنطقة العنقية بشكل مناسب.

تثبيت الـ Matrix: يقوم الدكتور بلف المقبض الموجود في الـ Tofflemire تدريجيًا حتى يصبح الـ Matrix Band متكيفًا حول السن بشكل جيد، مع تجنب الشد الزائد.

وضع الـ Wedge: يتم إدخال Wedge بين السن المراد ترميمه والسن المجاور، عادةً من المنطقة العنقية، لتثبيت المصفوفة وتحسين إغلاق الـ Gingival Margin والمساعدة في تكوين Contact Point.

التأكد من التكيف: يفحص الدكتور الـ Matrix Band من جميع الجهات للتأكد من عدم وجود فراغات عند الحافة العنقية وأن المصفوفة ثابتة ولا تتحرك.

وضع الـ Composite: بعد تحقيق العزل والتثبيت المناسب، يضع الدكتور مادة الـ Composite داخل التجويف على طبقات، ويقوم بتشكيل الجدار الجانبي مقابل الـ Matrix Band.

تشكيل الـ Contact Point: يساعد الـ Matrix Band والـ Wedge على إعادة بناء المنطقة الجانبية للسن وتكوين نقطة تماس مناسبة مع السن المجاور.

التصلب: يقوم الدكتور بعمل Light Cure لكل طبقة حسب تعليمات مادة الـ Composite.

إزالة الـ Tofflemire: بعد اكتمال الترميم وتصلب المادة، يقوم الدكتور بفك الـ Tofflemire Retainer ثم إزالة الـ Matrix Band والـ Wedge بعناية.

فحص الترميم: يتأكد الدكتور من وجود Contact Point مناسب، وعدم وجود Overhang أو زوائد، ثم يفحص الـ Margins والـ Occlusion ويقوم بالتشطيب والتلميع عند الحاجة.

كيف يستفيد منه الدكتور؟

يساعد الـ Tofflemire الدكتور على احتواء مادة الترميم أثناء ترميمات Class II، وإعادة بناء الجدار الجانبي للسن، وتكوين Contour مناسب. كما يوفر تثبيتًا جيدًا للـ Matrix Band أثناء وضع الـ Composite ويساعد على تقليل خروج المادة خارج حدود التحضير.

How to Use in Detail:

Select the matrix band: The dentist selects the appropriate Matrix Band according to the size and shape of the tooth. The band is positioned so that the wider portion is toward the gingival area and the narrower portion is toward the occlusal area.

Insert the matrix band: Insert the Matrix Band into the Tofflemire Retainer and adjust it so that it is securely held and its circumference can be controlled.

Try the matrix around the tooth: Position the Tofflemire around the tooth to ensure that the matrix band completely covers the missing proximal wall and extends adequately toward the cervical area.

Secure the matrix: Gradually tighten the Tofflemire retainer until the matrix band adapts closely around the tooth, avoiding excessive tightening.

Place the wedge: Insert a wedge between the tooth being restored and the adjacent tooth, usually from the cervical area. This stabilizes the matrix, improves the gingival seal, and helps establish a proper contact point.

Check adaptation: Inspect the matrix from all sides to ensure there are no significant gaps at the cervical margin and that the matrix is stable.

Place the composite: Once proper isolation and matrix stabilization are achieved, place the composite incrementally and build the proximal wall against the matrix band.

Form the contact point: The matrix band and wedge help recreate the proximal contour and establish an appropriate contact point with the adjacent tooth.

Cure the composite: Light-cure each composite increment according to the manufacturer’s instructions.

Remove the Tofflemire: After completing the restoration and curing the material, loosen the Tofflemire Retainer and carefully remove the matrix band and wedge.

Check the restoration: Verify the contact point, check for overhangs or excess material, and inspect the margins and occlusion. Perform finishing and polishing when required.
